Description of Benefits Included in the Plan:

Prescription – when you visit a participating pharmacy and show your card at the time of service you will receive an average savings of 20% on brand name and generic medications.

Eye Care - when you make an appointment at a participating eye-care professional and show your card at the time of service, you can receive 20% - 60% off the participating providers standard prices for services and eyewear.

Dental – when you make an appointment at a participating dental provider and show your card at the time of service, you will save 10% - 50% on your dental care expenses.

Chiropractic – when you make an appointment at a participating chiropractor and show your card at the time of service, you will receive 20% to 40% off the chiropractor's usual fee. Consultations are free.

Vitamins & Supplements - you can receive significant savings on vitamins and nutritional supplements with our online and mail order services.

How to Use the Membership

• Before you enroll, you can search for participating providers in your area by visiting www.findbestbenefits.com and entering in promo code 709102 for Program #1 or 709100 for Program #2. Or by calling 1-800-308-0374.
• To enroll, simply complete and detach the enclosed enrollment form and mail to: Plan Administrator 8770 W Bryn Mawr, Suite 1000 Chicago, IL 60631
• You should receive your membership kit within 5 to 7 business days from the date your application is received. Your membership kit will include your membership id card, an overview of each benefit and a listing of participating providers in your area.
• Once received, select a participating provider and schedule an appointment.
• Show your card to the participating provider at the time of service. The provider will adjust their pricing to reflect the discount. You will be responsible to pay for the discounted fee at the time of service.
• For prescriptions, simply visit a participating pharmacy and present your card and prescription to the pharmacists.
• The card can be used at any participating provider by you and your immediate family.

• This program is only available to residents of CT, MA, NJ, and NY.
